Overview

Melanoma is an aggressive form of skin cancer that develops from melanocytes — the pigment-producing cells responsible for skin colour. It is less common than other skin cancers but far more likely to spread (metastasize) if not detected early.

What Causes Melanoma?

The primary cause is damage to skin cells from ultraviolet (UV) radiation from the sun or indoor tanning.

Risk Factors

  • Excessive sun exposure

  • History of blistering sunburns

  • Fair skin, light hair, light eyes

  • Presence of multiple moles or atypical moles

  • Family history of melanoma

  • Weak immune system

  • Previous skin cancers

Melanoma can also occur in people with darker skin.


Common Symptoms

Melanoma can develop anywhere — commonly on the back, face, forearms, legs, and in rare cases, under nails or on mucosal surfaces.

Warning Signs – The ABCDE Rule

  • A – Asymmetry: One half does not match the other

  • B – Border: Edges appear irregular or blurred

  • C – Color: Multiple shades of brown, black, red, or blue

  • D – Diameter: Larger than 6 mm (pea-sized)

  • E – Evolving: Any change in size, shape, or symptoms (itching/bleeding)

Other Symptoms

  • New pigmented spot

  • Non-healing sore

  • Dark streak under a fingernail or toenail

  • Hard lump under the skin (advanced disease)

Any changing mole should be evaluated promptly.

Treatment Options

Treatment depends on stage, thickness (Breslow depth), and spread.

1. Surgery

Primary treatment for early-stage melanoma:

  • Wide local excision with safe margins

  • Sentinel lymph node biopsy to detect spread

  • Lymph node dissection (if nodes are involved)

2. Immunotherapy

Game-changing treatment for advanced melanoma.
Includes:

  • Pembrolizumab

  • Nivolumab

  • Ipilimumab

3. Targeted Therapy

Used for BRAF-mutated melanoma.
Common combinations:

  • Dabrafenib + Trametinib

4. Radiation Therapy
