Overview

Appendix cancer (appendiceal cancer) is a rare tumour that begins in the appendix—a small, finger-shaped organ attached to the large intestine. Unlike common gastrointestinal cancers, appendix cancer often causes no symptoms in early stages and is frequently discovered during surgery for appendicitis or abdominal scans.

Types of Appendix Cancer

1. Carcinoid (Neuroendocrine) Tumours

  • The most common type

  • Often slow-growing

  • Usually discovered incidentally

2. Adenocarcinoma

  • Arises from glandular cells

  • May behave like colon cancer

3. Mucinous Neoplasms

  • Produce mucin (jelly-like fluid)

  • Can lead to a condition called Pseudomyxoma Peritonei (PMP)

4. Goblet Cell Adenocarcinoma

  • A hybrid tumour with neuroendocrine and adenocarcinoma features

  • More aggressive

5. Signet Ring Cell Tumour

  • Rare and aggressive type


Common Symptoms

Many patients have no symptoms, but some may experience:

  • Appendicitis-like pain

  • Abdominal discomfort or bloating

  • Change in bowel habits

  • Unexplained weight loss

  • Nausea or vomiting

  • Abdominal swelling (in mucinous tumours / PMP)

Persistent or worsening abdominal symptoms require evaluation.


Risk Factors

Most cases have no identifiable cause, but risk increases with:

  • Age above 50

  • Genetic syndromes (MEN1, Lynch syndrome)

  • Smoking

  • Chronic inflammatory bowel conditions (rare)


Diagnosis at I D Cancer Center

Accurate diagnosis is essential for proper treatment planning.

1. Imaging Studies

  • CT scan of the abdomen

  • MRI (especially for mucinous tumours)

  • PET-CT for suspected spread

2. Endoscopy

  • Colonoscopy to rule out synchronous colon cancers

3. Lab Tests

  • Tumor markers: CEA, CA 19-9, CA-125

4. Histopathology

  • Surgical specimen or biopsy examination

  • Determines tumour grade and type

Treatment Options

1. Surgery (Primary Treatment)

Depending on tumour type and stage:

  • Appendectomy (removal of appendix)

  • Right hemicolectomy (part of colon removed)

  • Cytoreductive surgery for advanced mucinous tumours

  • HIPEC (Hyperthermic Intraperitoneal Chemotherapy)*
    *Performed at specialized centres with continued follow-up at I D Cancer Center.


2. Chemotherapy

Used for aggressive or advanced cancers:

  • IV chemotherapy for adenocarcinoma types

  • Specific protocols for goblet cell and signet ring tumours

  • May be used after surgery (adjuvant)


3. Radiation Therapy

While not commonly used for appendix tumours, radiation may help in:

  • Pain control

  • Localized recurrence

  • Palliative treatment
